Services provided by SAACP

SAACP provides four types of services:

  1. Education and Outreach
  2. Training
  3. Direct service
  4. Telephone Network Referral Care Services

 

  1. Education and Outreach: educational talks, workshop presentations and written materials are available for:
  • African American elders and their families
  • Caregivers (family and friends)
  • The Community at large (e.g. health fairs)

Topics include, but are not limited to:

  • Information regarding access to existing services in our region
  • How the health care systems works in the Seattle area
  • How to prepare a living will
  • How medical services are paid for
  • How to discuss concerns with your family members
  • Hospice; what is it and how does it work
  • Pain management

Educational sessions occur at senior centers, churches, or any gather place selected by those interested in learning more about end-of-life care.

  1. Training: Our goal is to provide trainings, or referral to trainings for caregivers, health care professionals (doctors, nurses, nurses aids, social workers and other care givers).

We currently offer, or offer referral to:

  • Participation in trainings at the UW Schools of Social Work, Nursing and the Medical School
  • We are adding now our completed training in 1, 2 and 4 hour modules for nurses and social workers serving African American patients facing life-threatening illness or end of life.

We are developing training for:

  • Physicians, nurses, long-term care staff and home care family or volunteers; regarding the specific cultural needs/vaules of African Americans facing end-of-life or a life-threatening illness
  • Clergy: Bereavement Workshop
  • Training for health care agencies who want to better serve the African American community

We create specific trainings specifically targeted at the needs of the person of agency requesting our services

  1. Telephone Network Referral Care Services. SAACP is partnering with a number of agencies in the greater Seattle area who already provide, or wish to do a better job in providing, culturally respectful services to African Americans. Every month, more partners are being added to our Referral Network.

You may access our Referral Service by calling the SAACP Office at 206-239-8189 during business hours, Monday through Friday. If you reach the answering machine, leave a message and your call will be returned as soon as possible.

The following are services we currently refer to (future service referral agencies we plan to add in the next 6 months are at the end of the list in italics):

  • Hospice home care
  • Inpatient hospice
  • Inpatient or outpatient palliative care (symptom management)
  • Long term care and nursing homes
  • Funeral and cemetery services
  • Personal funeral assistance
  • Compassion and Choices
  • Counselor referral
  • Physician referral
  • Legal
  • Financial
  • End-of-life care for those living with HIV/AIDS
  • Case management
